Before contacting us about your long term urine problem, we ask that you keep a Bladder Diary for 2-3 days. This helps us to find the cause of your urine problem. Print out copies of the form: Bladder Diary (opens in new tab) or make your own on blank paper. When you have filled out the diary, please submit our online form and attach a photograph or scan of your bladder diary.
